Intended purpose

Healthcare Notes provides educational explanations, structured prompts, ward orientation and tools that organise facts selected by the user. It is intended to support—not perform—professional nursing work.

The Dashboard formats nurse-selected findings into a draft. Results Lab teaches what tests measure and how patterns are commonly discussed. Neither function independently determines a diagnosis or treatment.

What Healthcare Notes is not

• Not an electronic medical record, medication chart or source of current patient orders.
• Not a diagnostic, pathology, triage, prescribing or emergency-response service.
• Not a substitute for observation, physical assessment, mental-state examination or escalation.
• Not a guarantee that documentation is complete, legally sufficient or accepted by an employer.
• Not a replacement for legislation, registration standards, scope of practice, local policy or approved clinical pathways.
• Not a system for entering identifiable patient results or confidential clinical records.

User responsibility

The user must assess the person, verify facts, distinguish observation from patient or collateral report, confirm current orders, apply the reporting laboratory’s reference interval, and follow local escalation criteria.

Generated text must be reviewed, corrected and completed before use. Never copy a statement merely because it appears in Healthcare Notes.

Pathology and Results Lab

Reference intervals, critical limits and test panels vary by laboratory, method, age, sex, pregnancy, clinical context and other factors. A result outside a reference interval does not by itself establish disease, and a result inside it does not exclude deterioration.

Results Lab uses non-patient educational patterns. It does not accept or interpret an identifiable patient report. Users must use the reporting laboratory, current orders, authorised clinicians and local high-risk-result communication pathways.

Emergencies and deterioration

Do not delay emergency action, rapid response, medical review, poison advice, security response or other escalation while using Healthcare Notes. If a person is deteriorating or at immediate risk, follow the current service pathway and emergency number.

Local policy and scope

A National Core resource is general Australian guidance. A hospital pack is local only when its status is shown as Verified Local with source, version and review information. If content conflicts with current local policy or an authorised direction, the local authority takes precedence.

Students and graduates must work within supervision and delegation arrangements. Membership does not expand scope or competence.

Content corrections and adverse concerns

Report suspected clinical inaccuracy, unsafe wording, outdated sources or harmful product behaviour immediately through the contact or complaint process. Include the page and wording, not patient information. Content may be restricted while reviewed.
